Consent for Medical Consultation Using Electronic Communication Medium by a Health Professional of Home + Physiotherapy
By utilizing Home + Physiotherapy’s telerehabilitation service, you provide consent for its health professionals to deliver consultation and rehabilitation services remotely (“telerehabilitation”). These consultations remain confidential at all times. It is your responsibility to ensure a stable Internet connection to prevent service interruptions and to be in a private setting to maintain the confidentiality of images and sound. Additionally, you agree to present a valid photo ID upon request at each appointment.
Despite Home + Physiotherapy’s efforts to maintain a reliable and secure connection, this technology has inherent limitations and risks, including potential loss of image and sound quality, as well as a risk of breaching confidentiality.
By opting for the telerehabilitation services of Home + Physiotherapy, you grant consent for the establishment of a patient file in the clinic’s software where the information transmitted during the consultation(s) will be documented. The clinical notes for each client will be securely kept confidential within the files of Home + Physiotherapy. It is also acknowledged that, while telerehabilitation offers a convenient means of consultation, the diagnostic assessment may be restricted without a physical examination, and an in-clinic consultation may be recommended in case of an unfavorable progression.
